Schedule
- Full-time, remote position within the U.S.
- Monday–Friday, 8:00 AM–5:00 PM or 9:00 AM–6:00 PM
- Must be able to work Pacific or Mountain Time hours
- Requires a stable home internet connection (speed test verification before interview)
What You’ll Do
- Support delivery of care to patients under the direction of Cadence Nurse Practitioners and clinical protocols
- Provide one-on-one coaching and support for seniors managing chronic conditions (e.g., type 2 diabetes, hypertension, cardiovascular disease)
- Conduct remote assessments of health status, lifestyle behaviors, and readiness for change
- Help patients follow their personalized care plans, focusing on behavior change, nutrition, physical activity, and self-management
- Monitor progress and adherence using regular check-ins and remote monitoring tools
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies
- Document interactions accurately and work closely with the clinical team to keep care proactive and coordinated
What You Need
- Active Medical Assistant certification from an accredited association (AAMA, AAH, AMT, NHA, NAHP, or NCCT)
- 5+ years of experience as a Medical Assistant
- Strong skills in behavioral-based coaching and patient education
- Solid clinical judgment and comfort assessing patients remotely within your scope
- Tech-savvy and able to work across multiple systems and tools
- Reliable, punctual, and comfortable working independently in a remote setting
- Experience in a high-growth or fast-paced environment while maintaining high clinical standards
- Excellent written, verbal, and interpersonal communication skills
- Prior experience supporting patients with chronic conditions (remote experience is a plus)
