What You’ll Do
- Provide one-on-one health coaching for patients managing chronic conditions (e.g. diabetes, hypertension, cardiovascular disease).
- Use motivational interviewing, SMART goals, and behavior change techniques to support sustainable lifestyle changes.
- Conduct regular check-ins using remote tools to track progress, adherence, and outcomes.
- Educate patients on disease management, medication adherence, symptom recognition, and preventive care.
- Build strong, supportive relationships that keep patients engaged and motivated.
- Troubleshoot barriers to care and escalate issues so patients get the right support at the right time.
- Share insights and feedback with the broader care team to improve care delivery and patient experience.
What You Need
- Active health coaching certification (NBHWC, NSHC, or ACE Advanced required).
- 1+ year of experience in health coaching or patient support, ideally focused on chronic condition management.
- Proven experience using behavior change strategies and motivational interviewing.
- Comfort working remotely with digital tools, care platforms, and databases.
- Strong written and verbal communication skills and a warm, patient-centered approach.
- Organized, dependable, and able to manage a steady caseload in a fast-paced, growth-focused environment.
- Experience with telehealth or remote patient support strongly preferred.
- Bonus: Experience in a healthcare startup or multidisciplinary, agile care environment.
