Schedule
- PRN, fully remote within the U.S.
- Minimum 4 shifts per month (as needed)
- Day, night, weekend, and holiday coverage
- Priority given to nights/weekends when offered
- At least 2 holidays per year
- Plus: 2 nights per quarter or 1 weekend per quarter if offered
What You’ll Do
- Respond to inbound patient clinical questions and escalations via phone and/or text
- Use standardized triage protocols (such as Schmitt–Thompson guidelines) to determine next steps, from non-urgent clinic follow-up to ER/911 in emergencies
- Monitor vitals, symptoms, and labs to identify patients needing clinical intervention and escalate appropriately
- Follow up on abnormal readings to assess clinical status and provide safe, timely triage
- Document all encounters and alert resolution clearly in the Cadence platform
- Create and manage tickets/follow-ups in Zendesk (training provided)
- Provide administrative support such as rescheduling, re-engaging patients with lapses in vitals, and other queue work as needed
- Ensure every patient interaction reflects exceptional, empathetic, patient-centered care in line with partner health system goals
What You Need
- Active RN compact multi-state license
- At least one additional RN license in CA, MI, IL, or AK (and willingness to add more as needed)
- 3+ years of experience treating chronic condition patients (CHF, hypertension, and/or diabetes) in inpatient or outpatient settings
- Confident using clinical triage protocols and making sound judgment calls remotely
- Comfortable working independently in a less structured, fast-changing environment
- Strong communication skills, calm phone presence, and ability to de-escalate and coach patients
- Passion for digital health, remote monitoring, and improving how patients experience chronic care
- Reliable, HIPAA-appropriate home setup with strong internet and a quiet workspace
