Schedule
- PRN, remote within the United States
- Minimum of 4 shifts per month, as needed
- Priority given to night shifts, and night shifts must be accepted when available
- Required coverage of 2 holidays per year
- Home office must have consistently strong Wi Fi with reliable upload and download speeds
What You’ll Do
- Serve as the clinical point person for remotely monitored patients with chronic conditions
- Oversee vitals, symptoms, and labs in partnership with RNs and MDs, responding to escalations from remote patient monitoring
- Manage treatment plans and medications, including initiating and titrating guideline directed medical therapy for heart failure along with evidence based treatment for Type 2 diabetes and hypertension
- Lead virtual check ins with patients to review labs, optimize GDMT, and reinforce adherence
- Support programs connected to both primary care and cardiology practices
- Ensure every interaction reflects exceptional, patient centered care and aligns with partner health system goals
- Contribute to internal growth efforts that help Cadence scale proactive care delivery for CHF and other chronic conditions
- Help shape team culture on one of the fastest growing groups within the company
What You Need
- Active compact multi state RN license
- Seven active NP licenses total, including TX plus at least six of the following: AL, AR, CT, FL, IL, IN, KY, MI, MS, NC, NY, OK, PA, TN, VA, VT, WI
- Master’s degree as a Nurse Practitioner
- ANCC or AANP certification
- 5 or more years of experience treating chronic disease patients with CHF, T2D, and hypertension in inpatient or outpatient settings at a high performing medical center
- Demonstrated experience managing heart failure using guideline directed medical therapy
- Experience with remote patient monitoring, CHF or T2D clinics, or startup environments is a plus
- HIPAA compliant, quiet home office with strong internet connection
- Comfort working independently with high trust, ownership, and direct feedback
- Strong clinical judgment, clear communication, and a passion for improving the patient experience through digital innovation
