Join our team and make a positive impact on the lives of others! We are looking for a Patient Forms Specialist to join our Pre-Care team.
This role requires travel into our Lake Success, NY or Orlando, Florida office on a quarterly basis.
Department & Position Overview:
Our program enables patients to hire someone they know and trust to care for them, and this role is crucial in helping them start on the program as quickly as possible.
Patients and caregivers have a variety of forms to complete and hoops to jump through before they are allowed to start the program. This role is crucial in helping that process be as efficient as possible, sending and tracking forms appropriately, interfacing with insurance companies and collaborating with colleagues on the Intake Team.
We aim for an exceptional experience for all our patients and caregivers, integrating technology as much as possible to eliminate paperwork and delays.
Every Day You Will:
Receive forms for patients and caregivers via email and fax and process those forms
Processing consists of uploading, logging, saving, and filing forms
Notify the necessary parties through verbal and written communication about form status and steps necessary to correct form
Determine the correct insurance company to send forms to and communicate updates with Intake representatives
Communicate with insurance representatives via phone, email, and fax to help forms move along
Complete training on our Intake process to better support efficiency and be on board with team goals
Support enrollment of new client cases across the entire Intake Department, sharing this responsibility with two others in a similar capacity
Ideal Candidate Will Possess:
Must be efficient and reliable
Exceptional attention to detail
Excellent written and verbal communication
Able to interface with insurance representatives with poise and patience
Supportive communication with colleagues on the Intake Team, helping them meet their monthly goals by filing forms in a timely manner
Proficient computer, phone knowledge. Intermediate to advanced typing skills
Experience with data input, scheduling and document uploading

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $18.00 and $23.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
