What You’ll Do
⦁ Outreach patients and caregivers to explain the benefits of home-based care (clinical, emotional, and logistical)
⦁ Build rapport with hesitant patients, address concerns, answer questions, and guide them toward enrollment
⦁ Use motivational interviewing and patient-centered communication to support decision-making
⦁ Review incoming referrals and conduct timely outreach to eligible patients
⦁ Manage enrollment end-to-end: eligibility checks, paperwork, and consent collection
⦁ Coordinate with care teams, discharge planners, physicians, and social workers to maintain continuity
⦁ Reconnect with undecided patients to continue relationship-building until they’re ready
⦁ Document outreach, barriers, and enrollment status accurately in EMR systems (Athena preferred)

What You Need
⦁ High school diploma or equivalent (associate or bachelor’s in a related field preferred)
⦁ 1–3 years in healthcare outreach, patient education, admissions, or another patient-facing role
⦁ Strong verbal and written communication with a persuasive, empathetic approach
⦁ Ability to explain medical and logistical info in plain, patient-friendly language
⦁ Confidence handling objections and navigating sensitive conversations
⦁ Comfort working in EMR systems (Athena a plus)
⦁ Familiarity with motivational interviewing or patient-centered communication (preferred)
⦁ Passion for helping vulnerable or underserved populations access care
⦁ Bonus: experience in home health, hospice, managed care, or chronic condition programs
⦁ Bonus: bilingual (especially Spanish)
