What You’ll Do

⦁ Engage and re-engage patients in Allara care programs through inbound and outbound outreach across phone, SMS, and live video calls

⦁ Support patients who have disengaged from care in reconnecting with their care team and help remove perceived barriers to follow-up

⦁ Provide empathetic, patient-centric support via phone, email, and in-app messaging with timely and accurate responses

⦁ Assist patients with scheduling, telehealth services, and general questions about their care plans and Allara’s offerings

⦁ Offer personalized assistance so patients can navigate their healthcare journey with confidence, compassion, and clarity

⦁ Ensure accuracy when relaying patient information between providers, patients, and internal teams

⦁ Stay current on Allara’s clinical and operational workflows to deliver seamless, informed support

What You Need

⦁ At least 1 year of experience in patient coordination, care management, or care navigation

⦁ Strong verbal and written communication skills with the ability to build rapport and navigate complex or sensitive conversations

⦁ Creative problem-solving skills to keep patient requests and follow-up moving smoothly

⦁ Solid technical skills and comfort working across multiple systems with sensitive patient data

⦁ A quiet, private workspace with reliable internet for confidential patient communications

⦁ Experience in digital health, telehealth, or virtual care, plus familiarity with Electronic Health Records (EHR) systems is preferred
