General Summary of Duties:
This position is responsible for obtaining patient information through a formal consent to release process, distributing medical information to the consented parties, and ensuring the integrity of protected health information (PHI).
Primary Occupational Responsibilities:
- Accurately obtains the patient’s demographic information and confirms an applicable consent to release of information document.
- Based on an informed consent and applicable federal, state, and local statutes, releases medical information to other authorized persons or agencies.
- Provides clerical assistance to administration during departmental audits and investigations. Answers all telephonic and electronic inquiries in a polite and timely manner.
- Places all received medical documents into the correct section of the patient’s record and with the proper naming convention.
- Maintains the integrity of PHI by adhering to governmental requirements and organizational policies and procedures.
- Responds to all medical release requests according to governmental and organizational standards.
- Expected to log into Zoom meeting if working remotely.
- Maintain or improve a 3 minute or better hold time for calls on queue and abandonment rate less than 3% daily.
EDUCATION:
Minimum Requirement of a High School Diploma.
EXPEREINCE:
Medical records experience (preferred)
KNOWLEDGE:
Knowledge of basic medical terminology and administrative processes. Knowledge of addressing and directing callers.
SKILLS:
Skill to exercise a high degree of diplomacy and tact while multi-tasking, organizing, and processing medical record requests. Skill to use the practice management module of electronic health record (EHR) platforms. Skill to speak clearly and professionally to callers and patients.
ABILITIES:
Ability to calm upset callers and patients via active listening and problem-solving activities. Ability to operate the practice management module of the EHR.
EQUIPMENT OPERATED:
Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.
PHYSICAL/MENTAL DEMANDS:
Must possess the physical and mental abilities to perform the tasks normally associated with the Medical Records Department. This involves sitting approximately 90% of the day with occasional standing, walking, reaching, and lifting. May encounter episodes of stress from handling a multitude of calls and medical record requests.
