Schedule
- Fully remote role within the United States
- Monday through Friday, 8 am–5 pm or 9 am–6 pm, aligned with Pacific or Mountain Time
- Full-time, phone- and video-based patient support and coaching
- Requires a stable home internet connection; a wifi speed test is completed before interview
What You’ll Do
⦁ Support delivery of remote healthcare under the direction of Cadence Nurse Practitioners and within Cadence clinical policies and procedures.
⦁ Provide one-on-one health coaching to patients managing chronic conditions, focusing on behavior modification, lifestyle changes, and self-management.
⦁ Conduct comprehensive assessments of patients’ health status, habits, and readiness to change.
⦁ Help patients follow and execute their personalized care plans, including nutrition, physical activity, and daily self-care strategies.
⦁ Monitor patient progress, adherence, and outcomes through regular check-ins and remote monitoring tools.
⦁ Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
What You Need
⦁ Active Medical Assistant certification from an accredited association (AAMA, AAH, AMT, NHA, NAHP, or NCCT).
⦁ At least 5 years of experience as a Medical Assistant.
⦁ Strong behavioral-based coaching skills and solid clinical judgment in patient assessment.
⦁ A proven track record of reliability, schedule adherence, and meeting role expectations.
⦁ Exceptional written, verbal, and interpersonal communication skills, with a patient-first mindset.
⦁ Experience educating and supporting patients with chronic conditions; prior remote support experience is a plus.
⦁ Tech-savvy and comfortable working with multiple digital systems in a high-growth, fast-paced environment.
