Schedule
- Full time, remote role
- Monday to Friday, 8:00 a.m. to 5:00 p.m. or 9:00 a.m. to 6:00 p.m.
- Must be able to work in Pacific or Mountain time zones
- Requires a quiet, professional home office with stable, high speed WiFi
- Wifi speed test required before interview to confirm connectivity
What You’ll Do
- Support delivery of remote health care under the direction of Cadence Nurse Practitioners and clinical policies
- Provide one on one coaching to seniors managing chronic conditions such as type 2 diabetes, hypertension, and cardiovascular disease
- Conduct comprehensive assessments of health status, lifestyle, nutrition, and readiness to change
- Help patients follow through on personalized care plans focused on behavior change, nutrition, physical activity, and self management
- Monitor patient progress, adherence, and outcomes using regular check ins and remote monitoring tools
- Educate patients on disease management, medications, symptom recognition, and prevention strategies
- Document patient interactions accurately while working across multiple technology systems
- Collaborate virtually with RNs, NPs, and the broader care team to keep care proactive and coordinated
What You Need
- Active Medical Assistant certification from an accredited association (AAMA, AAH, AMT, NHA, NAHP, or NCCT)
- At least 5 years of experience as a Medical Assistant
- Strong skills in behavioral based coaching and patient education for chronic conditions
- Solid clinical judgment and competency in patient assessment
- Prior experience in a high growth, fast paced environment while maintaining clinical standards
- Proven record of reliability, attendance, and schedule adherence
- Exceptional written, verbal, and interpersonal communication skills
- Comfortable working with minimal supervision and consistently punctual
- Tech savvy and confident navigating multiple systems and remote tools
- Experience supporting patients remotely is a plus
