Schedule
- Full-time, remote role
- Monday – Friday, 8:00 AM – 4:30 PM CST
- No weekends; rotational on-call approximately once per year
What You’ll Do
- Manage a patient census with a resolution-driven approach to close gaps in both clinical and non-clinical care.
- Identify and coordinate community resources to support patient care.
- Educate patients on chronic condition management and health literacy.
- Perform medication management, reconciliation, and coordination of refills.
- Coordinate delivery of critical services such as Home Health, DME, and Home Infusion.
- Build trust with patients, resolving questions and fostering open dialogue.
- Assist in managing referrals and scheduling appointments.
What You Need
- Active CCMA certification (required).
- At least 3 years of direct patient-facing experience.
- Strong communication skills, written and verbal.
- Proficiency with Microsoft Office Suite.
- Attention to detail and ability to manage multiple priorities.
