Schedule
Full-time, fully remote position based in Fairfax, VA. Monday–Friday with no weekends. Rotational on-call once per year. Flexible hours available to accommodate personal appointments.
What You’ll Do
- Manage a panel of patients, identifying and closing clinical and non-clinical care gaps.
- Coordinate community and home-based resources to enhance patient outcomes.
- Educate patients about managing chronic illnesses and maintaining medication adherence.
- Conduct medication reconciliation, identify potential issues, and coordinate refills.
- Ensure timely delivery of services including Home Health, DME, and Infusion.
- Manage referrals, schedule appointments, and maintain open, empathetic communication.
What You Need
- Active Washington D.C. LPN license and Compact/Multistate LPN license (required).
- At least 3 years of direct patient-facing healthcare experience.
- Excellent verbal and written communication skills.
- Strong technical proficiency in Microsoft Office Suite.
- Reliable high-speed Wi-Fi and a HIPAA-compliant home workspace.
- Bonus: Epic EHR experience, bilingual fluency, and additional state LPN licenses.
