- Full-time: $6.8K – $7.4K/month
- Part-time: $3.6K – $4.4K/month
What You’ll Do
- Support Medicare patients through complex health challenges
- Build trusted relationships through consistent, empathetic communication
- Help patients manage appointments, decisions, and care pathways
- Develop care plans addressing medical + social determinants (transportation, food, support at home)
- Navigate systems, remove barriers, and take action to solve problems
- Use Solace technology and tools to streamline care and patient outcomes
What You Need
- 3+ years in care management, advocacy, or healthcare navigation
- Deep understanding of Social Determinants of Health
- Strong clinical judgment and organizational skills
- Ability to manage multiple cases with clarity and urgency
- High comfort with technology and new software
- Heart, grit, and fierce advocacy for vulnerable patients
