Schedule
- Full-time, remote role within the United States
- Monday–Friday, 8:00 AM–5:00 PM or 9:00 AM–6:00 PM (local time)

What You’ll Do
- Provide one-on-one health coaching for patients managing chronic conditions (diabetes, hypertension, cardiovascular disease, and more)
- Use motivational interviewing, SMART goals, and behavior-change techniques to drive real health improvements
- Conduct regular check-ins using remote monitoring tools to track patient progress and adherence
- Educate patients on disease management, medication adherence, symptom recognition, and preventive care
- Build strong, supportive relationships that keep patients engaged and moving toward their wellness goals
- Troubleshoot barriers to care and help patients navigate challenges in their health journey
- Share feedback and insights to help improve care delivery processes and patient experience over time
What You Need
- Active health coaching certification (NBHWC, NSHC, ACE Advanced, or similar)
- 1+ year of experience in health coaching or patient support, ideally focused on chronic condition management
- Proven experience with behavior-change methods and motivational interviewing
- Comfort working in a fully virtual, tech-enabled environment with remote care tools and databases
- Strong communication skills with the ability to explain health concepts clearly and connect with both patients and clinical teams
- Solid organizational skills with the ability to manage a caseload and hit deadlines in a high-growth startup
- Problem-solver mindset with a proactive approach to identifying and addressing patient issues
- Experience with telehealth or remote patient care is strongly preferred
- Bonus: Prior experience in a healthcare startup or fast-moving clinical environment
