Schedule
- Full-time, remote role
- Must be able to work Mon–Fri, either:
- 8:00 a.m. – 5:00 p.m. Pacific, or
- 9:00 a.m. – 6:00 p.m. Pacific (or equivalent Mountain Time hours)
- Remote work from home within the U.S.
- Stable high-speed internet required (speed test completed before interview)
What You’ll Do
- Support delivery of remote healthcare under the direction of Cadence Nurse Practitioners and clinical policies
- Provide one-on-one coaching to seniors managing chronic conditions (type 2 diabetes, hypertension, cardiovascular disease, and more)
- Conduct comprehensive assessments of patient health status, lifestyle, nutrition, and readiness to change
- Help patients carry out personalized care plans focused on behavior change, nutrition, physical activity, and self-management
- Monitor patient progress and adherence through regular check-ins and remote monitoring tools
- Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies
- Document interactions accurately in the system and collaborate with the larger clinical team to keep care proactive
What You Need
- Active Medical Assistant certification from an accredited association (AAMA, AAH, AMT, NHA, NAHP, or NCCT)
- 5+ years of experience as a Medical Assistant
- Strong clinical assessment skills and comfort supporting patients with chronic conditions
- Proven experience in patient education and behavior-based coaching
- Track record of reliable attendance, schedule adherence, and meeting role expectations
- Excellent written, verbal, and interpersonal communication skills
- Ability to work independently in a remote setting and stay organized with multiple systems
- Tech-savvy and comfortable using digital tools and platforms
- Prior startup, high-growth, or fast-paced environment experience is a plus
- Experience supporting patients remotely is a plus
