Schedule
- Fully remote, full-time role
- Collaborate with clinical teams, referral partners, and families
- Fast-paced environment supporting continuity of care
What You’ll Do
- Deliver treatment updates to referral providers
- Coordinate discharge plans and aftercare referrals
- Communicate with clients, families, and providers to ensure smooth transitions
- Maintain accurate provider and referral information
- Follow up to confirm placement and monitor progress
- Document case management activity and collaborate with internal teams
- Meet performance and communication goals
What You Need
- Bachelor’s degree in psychology, social work, communications, healthcare, or related field
- 2+ years of healthcare or client-support experience (case management, referrals, admissions preferred)
- Strong communication and relationship-building ability
- Highly organized with excellent documentation skills
- Metrics-driven mindset with attention to detail
- Salesforce and Google Workspace/MS Office experience
- Authorized to work in the U.S.; fluent in English
- Ability to multitask and succeed in a fast-moving environment
