Responsibilities
- Serve as a primary bridge between clients, families, and referral providers
- Maintain consistent communication with hospitals, outpatient providers, schools, and community orgs
- Deliver professional treatment updates to referral partners
- Develop and document comprehensive discharge plans for clients completing treatment
- Identify appropriate aftercare resources and make accurate, timely referrals
- Track referral confirmations and follow up to verify placement
- Collaborate with internal clinical, admissions, and outreach teams
- Maintain precise documentation and progress notes
- Meet performance and service-quality targets
Requirements
- Bachelor’s degree in health sciences, psychology, social work, communications, or related field
- 2+ years relevant experience in healthcare, behavioral health, case management, referral coordination, or admissions
- Strong communication, empathy, and rapport-building skills
- Data-driven mindset with ability to work toward clear performance targets
- Extremely organized and detail-oriented in a fast-moving environment
- Salesforce and Google Workspace/MS Office proficiency
- U.S. work authorization and fluent English

Compensation & Benefits
- Base salary range: $52,500–$60,000
- Additional incentive and bonus potential
