- Remote, U.S. (role unavailable in AK, ME, DC, NJ, CA, NY, MA, CT, CO, WA, OR, MN)
- Afternoons/evenings required
What You’ll Do
- Be a primary point of contact for clients and referral providers
- Provide ongoing treatment updates to external partners
- Build and maintain trusting relationships with hospitals, schools, and clinicians
- Develop and coordinate individualized discharge plans
- Source and schedule aftercare referrals
- Confirm placement and follow up to ensure continuity of care
- Maintain accurate documentation and case notes
- Collaborate with internal clinical and admissions teams
- Track and hit performance metrics
What You Bring
- Bachelor’s degree in psychology, social work, health sciences, or related field
- 2+ years in healthcare, case management, discharge coordination, admissions, or similar roles
- Excellent communication skills, written and verbal
- Empathy, professionalism, and strong relationship-building instincts
- CRM proficiency (Salesforce preferred)
- Organized, detail-driven, able to juggle multiple priorities
- Goal-oriented mindset with experience meeting concrete targets
- Authorized to work in the U.S.
Compensation & Benefits
- Salary range: $52,500 – $60,000
