Schedule
- Fully remote (U.S.)
- Full-time role coordinating client transitions and provider communications
- Collaborate closely with clinical, admissions, and outreach teams
What You’ll Do
- Provide treatment updates to referral partners and answer provider questions
- Develop comprehensive discharge plans and aftercare referrals
- Maintain and use a nationwide provider database
- Communicate with clients, families, and external providers to ensure smooth transitions
- Document case notes and collaborate with clinical and case management teams
- Follow up to confirm placements and ensure continuity of support
- Meet performance and communication metrics
What You Need
- Bachelor’s degree in health sciences, psychology, social work, communications, or related field
- 2+ years relevant healthcare experience (case management, discharge planning, referrals, outreach)
- Strong communication and relationship-building skills
- Detail-oriented, organized, and metrics-driven
- Salesforce and Google Suite/MS Office experience
- Authorized to work in the U.S.; native/bilingual English proficiency
- Ability to excel in fast-paced, mission-driven work
