What You’ll Do
- Provide treatment updates to referral sources such as hospitals, schools, and outpatient practices
- Build trusting relationships with providers and ensure clear, professional communication
- Develop and coordinate discharge plans to support client success post-treatment
- Identify appropriate aftercare resources using a nationwide provider database
- Make accurate referrals and follow up to confirm placement and smooth transitions
- Document case management contacts and update treatment teams on discharge planning progress
- Collaborate with internal teams (clinical, admissions, partnerships) to ensure cohesive support
What You Need
- Bachelor’s degree in health sciences, psychology, social work, communications, or related field
- At least 2 years of healthcare or case management experience (discharge planning, referral relations, outreach, or admissions)
- Strong interpersonal, communication, and relationship-building skills
- Highly organized, detail-oriented, and results-driven
- Proficiency with Salesforce and Google Suite/MS Office
- Ability to thrive in a fast-paced environment and meet performance targets
- U.S. work authorization with native or bilingual English proficiency
