This is for somebody who can be calm, clinical-adjacent, and organized while juggling a lot of moving pieces. You’re basically the glue between Charlie Health, the client and family, and outside providers, making sure updates land clean and discharge plans don’t fall through the cracks.

What You’ll Do
⦁ Provide consistent treatment updates to referral sources (hospitals, outpatient practices, schools, government orgs)
⦁ Build strong relationships with referral partners, answering questions and keeping communication smooth and professional
⦁ Lead discharge planning for clients completing treatment, ensuring plans are comprehensive and clinically appropriate
⦁ Identify aftercare resources and make referrals using each provider’s preferred channels
⦁ Maintain and use a nationwide provider database to find the right aftercare options
⦁ Follow up to confirm placement and continuity of care
⦁ Document case management contacts in progress notes and keep internal teams updated on discharge status
⦁ Coordinate with therapists, case managers, admissions, and other internal stakeholders as needed
⦁ Partner with Clinical Outreach and Partnerships to deepen your understanding of referral sources and services
⦁ Follow policies, procedures, and hit performance metric goals
What You Need
⦁ Bachelor’s degree in health sciences, communications, psychology, social work, or related field
⦁ 2+ years relevant experience (case management, discharge planning, referral relations, admissions, outreach, or similar patient-facing healthcare work)
⦁ Strong relationship-building and listening skills
⦁ Metrics-driven mindset with experience working toward clear targets
⦁ Excellent written and verbal communication
⦁ Extremely organized with high attention to detail
⦁ Work authorized in the U.S. with native or bilingual English proficiency
⦁ Comfortable in a fast-paced environment and able to learn quickly
⦁ Proficiency in Salesforce plus Google Suite / MS Office
Benefits

⦁ Base pay range: $52,500–$60,000/year (final depends on location and experience)
⦁ Total comp may include incentives, bonuses, and other benefits depending on the role
