What You’ll Do
- Provide treatment updates to referring providers (hospitals, outpatient practices, schools, government orgs) and respond to questions about referred clients
- Build strong, professional relationships with referral sources to keep coordination smooth and trusted
- Develop comprehensive, clinically appropriate discharge plans that set clients and families up for success post-program
- Identify aftercare resources and make accurate, timely referrals for clients discharging from Charlie Health
- Maintain and use a nationwide provider database to find the best-fit referral options
- Send referrals through each provider’s preferred communication channel and follow up to confirm placement
- Ensure receiving providers have all required clinical materials and referral information
- Document case management contacts in progress notes and communicate discharge status to therapists, case managers, and the treatment team
- Partner closely with internal teams (clinical, admissions, outreach, partnerships) to meet client needs and performance goals
- Follow policies and hit performance metrics consistently
What You Need
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- 2+ years of relevant experience (case management, discharge planning, referral relations, admissions, outreach, or other patient-facing healthcare work)
- Strong relationship-building and listening skills
- Metrics-driven mindset and experience working toward concrete targets
- Excellent written and verbal communication
- High organization and attention to detail
- Authorized to work in the U.S. with native or bilingual English proficiency
- Comfortable in a fast-paced environment and quick to learn
- Proficiency with Salesforce plus Google Suite and/or Microsoft Office
Benefits

- Expected base pay: $52,500–$60,000 per year (final pay varies by location and experience)
