Full-time, remote role based in the United States.
This role is not available to candidates in Alaska, Maine, Washington DC, New Jersey, California, New York, Massachusetts, Connecticut, Colorado, Washington State, Oregon, or Minnesota.
You’ll work in a fast-paced, metrics-driven environment, coordinating with internal teams, external providers, and families across multiple time zones.
What You’ll Do
- Build supportive, professional relationships with referral sources such as hospitals, outpatient providers, schools, and community organizations.
- Provide timely treatment updates to referring providers, answer questions about their referred clients, and ensure they have all necessary clinical information.
- Work directly with clients and families to understand their needs, preferences, and goals as they approach discharge.
- Develop comprehensive, clinically appropriate discharge plans that connect clients to the right aftercare resources.
- Use and maintain a nationwide provider database to identify and recommend appropriate aftercare options.
- Make referrals using each provider’s preferred communication channels and follow up to verify placement.
- Document all case management contacts and updates in progress notes and communicate with therapists, case managers, and treatment teams about discharge status.
- Collaborate closely with internal teams (Clinical, Admissions, Outreach, Partnerships, etc.) to coordinate care and support continuity.
- Partner with Clinical Outreach and Partnerships teams to deepen understanding of referral sources and the services they offer.
- Adhere to policies, procedures, and performance metrics to consistently deliver high-quality, reliable coordination.
What You Need
- Bachelor’s degree in health sciences, communications, psychology, social work, or a related field.
- At least 2 years of relevant experience in healthcare or behavioral health, preferably in client-facing roles such as case management, discharge planning, referral relations, admissions, or outreach.
- Strong interpersonal, relationship-building, and active listening skills.
- Metrics- and results-oriented mindset, with experience working toward clear performance targets.
- Excellent written and verbal communication skills.
- Exceptional organization and attention to detail.
- Work authorization in the United States and native or bilingual English proficiency.
- Ability to thrive in a fast-paced environment and learn quickly.
- Proficiency with Salesforce and Google Suite/Microsoft Office.
Benefits

- Expected base pay between $52,500 and $60,000 per year, depending on location and years of experience.
- Total compensation may include incentive pay, discretionary bonuses, and other short- and long-term incentive opportunities, plus additional Charlie Health–sponsored benefits.
