Schedule
- Full-time, remote
- May require up to four annual visits to Columbia, MD for company events
- Priority consideration for candidates in Florida, Illinois, and Kentucky
- Requires reliable technology and consistent collaboration with a remote clinical team
Responsibilities
- Provide telephonic consultation for assigned patient cohorts and assess clinical disposition
- Engage non-compliant members and support reactivation into care
- Collaborate with clinical services, health coaching, and member support teams
- Build strong working relationships across the clinical team
- Facilitate or delegate patient education aligned with best practices and standards of care
- Assist members with mobile care management devices
- Maximize member access to available programs and resources
- Support closing gaps in care and maintaining high adherence to care plans
- Document all interactions with accuracy in Vheda’s platform
Requirements
- Active medical certification (MA, CNA, MedTech, LPN, Health Coach, Case Management, etc.)
- 1–3 years of experience in a medical care setting
- General knowledge of chronic disease management (Diabetes, Hypertension, CHF, Asthma, COPD, Weight Management, Behavioral Health)
- Proficient with web-based applications and technology
- Strong communication skills with compassion, empathy, and cultural awareness
- Proven remote work capability and collaboration skills
- Excellent interpersonal, organizational, and communication abilities
Preferred
- Experience in urgent care, emergency settings, or front/back office clinical roles
- Experience working with diverse populations
- Knowledge of Medicaid and/or Medicare
- Telephonic provider or patient outreach experience
- Familiarity with HEDIS and/or NCQA standards
- Spanish fluency a plus
