Schedule
Full time, remote role. Must be based in Eugene, Oregon or within a commutable distance. Not available to candidates in: AK, ME, Washington DC, NJ, CA, NY, MA, CT, CO, WA, OR, or MN.

What You’ll Do

⦁ Create a supportive, positive experience for clients and referral sources or external providers

⦁ Work directly with clients, families, and referral sources to understand needs, preferences, and next step options

⦁ Make accurate, timely outbound referrals for clients who are not admitted to Charlie Health

⦁ Collaborate with internal teams (clinical, admissions, outreach, partnerships) to deliver a smooth, coordinated experience

⦁ Document all interactions clearly in the electronic record system and maintain strong follow through

⦁ Build a deep understanding of referral sources and services through partnership with Outreach and Partnerships teams

⦁ Serve as a liaison between Charlie Health and partners to ensure ongoing needs are met and client experience stays central

⦁ Follow policies and procedures while meeting performance metrics goals and KPI targets

What You Need

⦁ Bachelor’s degree in health sciences, communications, psychology, social work, or a related field

⦁ 1–2 years of relevant experience in healthcare or patient facing roles (case management, discharge planning, referral relations, admissions, outreach, or similar)

⦁ Strong relationship building, listening, and communication skills

⦁ Metrics driven mindset, with a track record of meeting or exceeding KPIs

⦁ Excellent organization, documentation habits, and attention to detail

⦁ Work authorization in the U.S. plus native or bilingual English proficiency

⦁ Comfort in a fast paced environment with the ability to learn quickly

⦁ Proficiency in Salesforce and Google Suite or Microsoft Office
