What You’ll Do
- Respond to patient and durable power of attorney (DPOA) phone inquiries with professionalism and empathy.
- Handle patient and caregiver messages sent through the Curana Patient Portal.
- Assist patients with scheduling follow-up appointments with Curana providers and specialists.
- Manage and update electronic health records (EHR) to keep patient information accurate and current.
- Coordinate documents for provider review and signature and help facilitate provider orders.
- Maintain patient rosters for programs such as Advanced Primary Care Management (APCM) and Guiding an Improved Dementia Experience (GUIDE).
- Support provider scheduling, assist with prior authorizations, and obtain patient records and diagnostic test results.
- Answer and manage incoming calls, collaborating with the Curana Interdisciplinary Care Team to keep communication smooth across the network.
- Perform other related duties as assigned to support patients, providers, and care managers.
What You Need
- High school diploma or equivalent.
- At least 1 year of experience in a medical office, senior living community engagement, or a related field.
- At least 1 year of experience with Electronic Health Record (EHR) documentation or practice management tools.
- Strong understanding of medical terminology and the ability to interpret medical records, lab results, and appointment notes.
- Basic knowledge of patient screening tools and the ability to notice changes over time.
- Excellent organizational and time management skills, with the ability to prioritize and handle multiple tasks at once.
- Ability to work in a quiet, distraction-free home environment.
- Proficient computer skills and comfort learning new virtual platforms and tools.
- Preferred: prior experience with virtual triage and bilingual or multilingual communication skills.
