What You’ll Do
- Support care navigation for patients, including finding in-network providers and connecting patients to social services (housing, transportation, WIC, SNAP)
- Monitor the member support phone line and answer patient questions
- Complete eligibility checks for new and existing patients and own the offboarding process
- Own patient scheduling, including prioritization, appointment reminders, and provider schedule optimization
- Manage referrals to the health plan’s Case Management team
- Partner with the Market Operations Lead to build and improve workflows that increase efficiency
- Handle additional operational tasks as assigned
What You Need
- 4+ years of healthcare administration experience (virtual care experience preferred)
- Familiarity with SDOH platforms and referral workflows
- Strong attention to detail and comfort with ambiguity
- Able to work cross-functionally with clinicians
- Results-oriented, fast-moving operator mindset
- Patient-first energy and genuine interest in maternal and child health
