Schedule
Full-time, remote position within the United States. Monday–Friday, 8:00 a.m.–4:30 p.m. CST. No weekends. Rotational on-call once per year on average.
What You’ll Do
- Manage a panel of chronic care management patients with a focus on closing clinical and non-clinical care gaps.
- Coordinate community resources, home health, and other critical services to support patient care.
- Provide patient education on chronic condition management and medication adherence.
- Conduct medication reconciliations, manage refills, and identify potential concerns.
- Assist with referrals, appointment scheduling, and care follow-up.
- Foster open dialogue with patients to understand their needs and build trust.
What You Need
- Active CCMA certification (required).
- At least 3 years of direct, patient-facing healthcare experience.
- Excellent written and verbal communication skills.
- Proficiency with Microsoft Office Suite; technical aptitude for remote work.
- Reliable high-speed internet and a HIPAA-compliant home workspace.
- Bonus: Experience with Epic and/or bilingual fluency.
