Schedule

What You’ll Do
- Serve as the main point of contact for patients, providing support and education throughout their care journey
- Coordinate and schedule appointments, follow-ups, and referrals between patients and healthcare providers
- Respond to patient inquiries with empathy and professionalism
- Ensure accurate and timely documentation of all communications in the system
- Partner with healthcare professionals to build and manage personalized care plans
- Monitor patient progress and address any barriers to continued care
What You Need
- Experience in a healthcare, care coordination, or patient support role preferred
- Excellent communication and organizational skills
- Ability to multitask in a fast-paced virtual environment
- Comfort with healthcare management software or similar systems
- Empathy, patience, and a genuine desire to help others succeed
