About Pair Team
Pair Team is a tech-enabled medical group reimagining how Medicaid and Medicare serve the country’s most underserved populations. They partner with clinics, community-based organizations, and health systems to deliver whole-person care that addresses clinical, behavioral, and social needs. Their model is already showing real impact, including major reductions in emergency department visits and hospital admissions for high-need patients.
Schedule
- Full-time, remote position based in the United States
- Core schedule: Wednesday–Sunday, with Mondays and Tuesdays off
- Required to work Pacific Time (PST) hours
- First 30 days: training Monday–Friday during PST business hours
- Requires a quiet, HIPAA-compliant, internet-connected workspace at home
What You’ll Do
- Identify, outreach, engage, and enroll eligible individuals (including those with Serious Mental Illness, Substance Use Disorder, high medical needs, and/or experiencing homelessness) into Pair Team’s Enhanced Care Management program via phone, text, email, and in-person events
- Build trust with hesitant or previously underserved patients by listening deeply, meeting them where they are, and clearly explaining the benefits of whole-person care
- Educate clinic, community-based, and health system partners on Pair Team’s services and how to connect eligible members
- Manage the full enrollment funnel from first contact through successful enrollment, monitoring delays and blockers and keeping internal teams and partners updated
- Meet and exceed enrollment and outreach metrics while maintaining high-quality, compassionate interactions
- Coordinate effectively with internal care teams and external partners to ensure seamless communication and handoffs
- Use multiple technology platforms, tools, and systems to track outreach, document interactions, and monitor funnel performance
What You Need
- Currently reside in the United States and able to work remotely in PST hours
- Bilingual in English and Spanish, with strong reading, writing, speaking, and listening skills in both languages
- At least 1 year of experience in a metric or quota-driven role (outreach, sales, enrollment, customer service, or similar)
- Proven track record of being results-driven and consistently meeting or exceeding performance targets
- Strong technical comfort using multiple platforms, CRMs, and communication tools
- A quiet, HIPAA-compliant home workspace with reliable internet
- Valid driver’s license and auto liability insurance for occasional in-person outreach or partner visits
Preferred
- Experience with high-volume outreach and engagement campaigns
- Background working with underserved communities, social services, healthcare, or behavioral health
- Exceptional listening skills and ability to “read the room” and build rapport with people who may distrust the healthcare system
- Reliable, flexible, and comfortable with change in a fast-evolving, mission-driven environment
