Help at Home provides home care services that help clients live independently and safely in their own homes. This position supports care coordination and service delivery, acting as a bridge between clients, caregivers, and internal teams to ensure quality care and compliance.

What You’ll Do
- Maintain client files and related documentation while protecting confidentiality under HIPAA
- Assign homecare aides based on client needs and Plan of Care
- Coach caregivers to support high-quality care and escalate performance concerns as needed
- Communicate schedule updates and changes with clients, aides, referral sources, and management
- Handle client complaints, document issues, and report significant concerns (mandatory reporter for critical incidents)
- Conduct in-home visits when needed to assess needs and support relationship-building
- Track client satisfaction surveys and document follow-ups on areas of concern
- Prepare and submit routine departmental reports
- Document client condition, service updates, and changes to Plan of Care
- Ensure compliance with local, state, and federal laws and company policies
- If supporting Developmental Disabilities (DD), follow state-specific training, documentation, and meeting requirements
What You Need
- Bilingual in Spanish and English (read, write, speak)
- At least 18 years of age
- Strong organization and multitasking skills
- Strong customer service and communication skills
- Calm, professional conflict resolution and problem-solving ability
- Basic computer literacy and typing skills
- Compassionate, responsible approach to client care and caregiver support
- High school diploma or equivalent preferred (may require higher education/certification depending on location)
- Medicaid, Waiver, or Home Healthcare experience preferred
- Current PPD or chest X-ray if applicable
- Ability to move through offices and client homes, including stairs; may require lifting/moving up to 50 pounds (depending on assignment)
