Pair Team is a mission driven, tech enabled medical group focused on whole person care for Medicaid and Medicare members. They partner with community based organizations, clinics, and social service agencies to coordinate medical, behavioral, and social support for people living with serious mental illness, substance use disorder, homelessness, and high medical needs.
Schedule
- Full time, remote role
- Must be able to work Pacific Time schedule
- Mix of work from home plus occasional on site engagement days in supported communities
- Standard Monday to Friday daytime hours
What You’ll Do
- Manage up to six interdisciplinary pods and support up to 18 Lead Care Managers at a time
- Provide one to one coaching, performance management, and day to day support for your team
- Audit documentation and care plans on a regular basis to ensure quality, clarity, and timely follow up
- Monitor dashboards and KPIs for care engagement, outreach, and patient outcomes, and act quickly when metrics slip
- Serve as first point of contact for escalations from Lead Care Managers, clinics, and partners
- Own and update SOPs and help design and deliver training for new and existing team members
- Coordinate closely with clinics, community partners, and internal teams to align on care plans and support services
- Identify process gaps and recommend improvements to care management and coordination workflows
What You Need
- At least 2 years of experience directly managing a large team of individual contributors, ideally 10 or more people
- 2 or more years of experience in community engagement, patient navigation, case management, or social work
- Background working in multidisciplinary or collaborative care models
- Fluency in both English and Spanish
- Experience working with individuals facing complex needs such as homelessness, serious mental illness, or substance use disorder
- Strong cultural fluency and knowledge of community resources
- Comfortable using CRM or care management tools plus basic Excel, Word, email, and video platforms
- Quiet, HIPAA compliant home workspace with reliable internet and phone access
- Detail oriented, organized, and able to multitask in a fast changing environment
