Schedule
- Full-time, remote role based in the United States
- Must work Pacific Time (PST) hours
- Mix of remote work with some in-person engagement in local communities as needed

What You’ll Do
- Lead and develop a team of up to 18 Lead Care Managers across six interdisciplinary pods, building trust, providing one on one coaching, and driving high performance
- Conduct regular quality audits of care manager work to ensure timely documentation, accurate tasks, and personalized care plans for individuals with complex medical and social needs
- Monitor performance dashboards and key metrics, serving as the first point of contact for escalations, troubleshooting, and course correction
- Support hiring, onboarding, and ongoing training for Lead Care Managers, including participation in interviews and maintaining up-to-date Standard Operating Procedures
- Serve as a primary liaison between LCMs, assigned clinics, community partners, and cross functional teams to promote strong communication and collaborative care
- Identify opportunities to improve care management processes and coordination, and clearly communicate recommendations to the broader healthcare team
- Directly support outreach and engagement strategies for individuals living with Serious Mental Illness, Substance Use Disorder, homelessness, and high medical needs, ensuring they are connected to Enhanced Care Management services
- Model Pair Team’s values in daily work, reinforcing a culture of integrity, collaboration, and patient centered care
What You Need
- At least 2 years of experience directly managing large individual contributor teams, ideally in healthcare, behavioral health, or community care
- 2 or more years of experience in community engagement, patient navigation, case coordination, or social work
- Experience working within a multidisciplinary, collaborative care model
- Bilingual in English and Spanish, with strong verbal and written communication in both languages
- Strong cultural fluency and knowledge of community resources that support individuals with complex needs
- Demonstrated experience serving populations facing homelessness, Serious Mental Illness, Substance Use Disorder, or chronic medical conditions
- Technical proficiency with CRM databases, basic Excel, Word, email, and video conferencing tools
- Quiet, HIPAA-compliant home office setup with reliable internet and cell phone access
Preferred
- Professional or personal lived experience working closely with individuals experiencing homelessness, complex chronic conditions, or Severe Mental Illness/Substance Use Disorder
- 2 or more years of case management experience
- Experience using motivational interviewing
- Working knowledge of medical terminology
- Strong problem solving mindset, detail oriented, organized, and comfortable juggling multiple priorities
- Reliable and adaptable in a fast changing, mission driven environment
