Overview
M Health Fairview has an opening for an Advance Care Planning Liaison in the Honoring Choices department supporting and facilitating patient completion of Advance Care Planning (ACP) documentation for the “I CAN DO Surgical ACP” NIH-funded multi-site pragmatic, randomized trial (PI: Genevieve Melton-Meaux). The I CAN DO Surgical ACP collaborative includes the University of California – San Francisco, the University of Minnesota, and the University of California – Irvine. This trial will evaluate the effectiveness of incorporating evidence-based ACP tools into the pre-surgery workflow and ultimately help older patients receive treatments that fit their goals and values, even when they are unable to make their own medical decisions.
Since this is a grant-funded role, preference will be given to LSW candidates.
This opening is remote, however, there are on-site requirements therefore best-suited for candidatres in the Twin Cities metro area.
This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible opening.

Responsibilities/Job Description
The Health Care Navigator position involves providing support, guidance, and assistance for patients and families as they engage in Advance Care Planning (ACP). Our primary goals are to increase documention of ACP in the medical record as well as increasing patient satisfaction, and optimizing care and outcomes. The HCN will act as a communication liaison to understand the patient’s individual needs, values, goals, and preferences for medical care. They will also assess readiness of patients to engage in ACP, guide the patient and the surrogate in reflecting on patients’ values and treatment preferences, help the patient designate surrogate decision makers, and complete advance directives, if desired. The HCN will also work in partnership with patients to assist them in communicating their goals, values and treatment preferences to their surrogates and clinicians, and ensure these preferences are documented in the electronic health record.
Qualifications
Required
Education:
One of the following degrees:
- BA or BS
Experience:
One year demonstrated knowledge/experience in:
- Working both independently and within a collaborative team while achieving goals
- Utilizing organizational and communication skills
- Building relationships with system teams to achieve specified outcomes
Proficiency in Microsoft applications
Proficiency in Epic application
Preferred
Education
- Nursing, Social Work, or related medical field
- This is a grant-funded role and the ideal candidate will be a licensed Social Worker
Experience
- Knowledge of the purpose and process of Advance Care Planning
- Knowledge of ACP process and resources within M Health Fairview, Ebenezer, or similar health care systems
- Implementation of new processes in a healthcare organization
- Teaching in small and large groups
