Schedule
- Full-time, hourly (non-exempt)
- Remote
- Compensation: $17–$19 per hour
What You’ll Do
- Call eligible patients and clearly explain the benefits of care programs for chronic condition management
- Support the care team with patient communication and follow up
- Verify eligibility and confirm insurance coverage
- Maintain accurate records of patient interactions and documentation to support the patient chart
- Communicate with empathy and professionalism to build trust and encourage participation
- Coordinate with internal teams and healthcare providers to ensure smooth handoffs and support
- Meet or exceed enrollment performance targets consistently
What You Need
- Preferred: prior experience in a healthcare setting
- Strong computer skills and comfort with data entry
- Strong problem-solving skills and ability to multitask
- Excellent verbal and written communication
- Positive, collaborative team mindset
- Ability to work independently in a remote environment
- Ability to pass a background check and drug screening
- Experience with multiple EMR systems and payer portals
