This is a clinical care management role where you’ll work telephonically with CareFirst members to build care plans, reduce avoidable admissions, and improve outcomes through strong case management and motivational interviewing. If you’re an RN who can connect with people, stay organized, and document cleanly, this role is built for that skill set.

Schedule

What You’ll Do

Engage members, caregivers, and providers by phone to assess needs and create comprehensive plans of care

Collaborate with PCPs, specialists, and the interdisciplinary care team to meet member goals

Reassess and update care plans as conditions, preferences, and care settings change

Coordinate patient education using the best modality for each member

Evaluate social determinants of health, identify benefits and community resources, and coordinate referrals

Help coordinate tests, imaging, and specialist consults when needed

Perform medication reconciliation at the start of the care plan, after changes, and every 30 days, monitoring interactions and side effects

Support transitions of care and post-discharge follow-up to reduce readmissions

Document consistently to reflect progress, barriers, and outcomes using established standards

Meet productivity and quality metrics, complete mandatory training and annual competencies, and participate in team huddles and clinical learning

What You Need

Current multi-state compact RN license in your state of residence, with ability to obtain additional licenses (BSN preferred)

3–5 years of varied clinical experience (telephonic case management strongly preferred)

Strong motivational interviewing and care management skills (training in MI preferred)

High organization, problem-solving ability, and comfort working in a fast-paced environment

Strong documentation skills, including care plan documentation and summarizing medical records into clear, concise “member stories”

Computer proficiency with EMRs and tools like Microsoft Teams, Word, Excel, and Outlook, plus ability to learn custom applications

Ability to maintain HIPAA compliance in a home office setting
