What You’ll Do
- Provide consistent, high-quality treatment updates to referring providers and answer questions about referred clients
- Build trusting relationships with referral sources (hospitals, outpatient practices, schools, government orgs) through professional, collaborative communication
- Create comprehensive discharge plans that set clients and families up for success post-program
- Identify aftercare resources and make accurate, timely referrals for clients discharging from Charlie Health
- Maintain and use a nationwide provider database to find best-fit referral options
- Send referrals via each provider’s preferred channel and follow up to confirm placement
- Ensure receiving providers have all required clinical materials and information to review/accept referrals
- Document case management contacts in progress notes and keep therapists/case managers/treatment team updated on discharge status
- Collaborate with internal partners (clinical, admissions, outreach, partnerships) to keep coordination tight
- Follow policies and hit performance metrics consistently
What You Need
- Bachelor’s degree in health sciences, communications, psychology, social work, or related field
- 2+ years of relevant experience (case management, discharge planning, referral relations, admissions, outreach, or patient-facing healthcare work)
- Strong relationship-building and listening skills
- Metrics-driven mindset with experience working toward concrete targets
- Excellent written and verbal communication
- Highly organized, detail-obsessed (in the good way)
- U.S. work authorization and native or bilingual English proficiency
- Ability to thrive in a fast-paced environment and learn quickly
- Proficiency with Salesforce plus Google Suite and/or Microsoft Office
Benefits

- Base pay: $52,500–$60,000 (varies by location and experience)
