What You’ll Do
⦁ Ensure a supportive, positive experience for clients and referral sources
⦁ Work directly with clients, families, and referral partners to understand needs and preferences
⦁ Make accurate, timely outbound referrals for clients who are not admitted to Charlie Health
⦁ Collaborate with internal teams like clinical and admissions to coordinate next steps
⦁ Document all interactions in the electronic record system
⦁ Partner with Clinical Outreach and Partnerships teams to understand referral sources and services
⦁ Act as liaison between Charlie Health and partner providers to keep client experience front and center
⦁ Follow policies, procedures, and performance metrics tied to admissions and referral outcomes

What You Need
⦁ Bachelor’s degree in health sciences, communications, psychology, social work, or related field
⦁ 1–2 years of relevant experience (case management, discharge planning, referral relations, admissions, outreach, or similar patient facing work)
⦁ Strong relationship building and listening skills
⦁ Metrics driven mindset with experience working against targets and KPIs
⦁ Excellent written and verbal communication skills
⦁ Strong organization and attention to detail
⦁ Work authorization in the U.S. and native or bilingual English proficiency
⦁ Ability to learn quickly in a fast paced environment
⦁ Proficiency with Salesforce and Google Suite or Microsoft Office
