About Pair Team
Pair Team is a tech-enabled medical group focused on improving how Medicaid and Medicare serve underserved populations. They deliver whole-person care (clinical, behavioral, and social) and use AI and automation to reduce admin work so care teams can focus on people. Their model supports high-need patients and is built around community-driven partnerships and measurable impact.

Schedule
• Full-time, Remote (United States)
• Set schedule: Wednesday through Sunday (days off Monday and Tuesday)
• Must work Pacific Time hours
• Training for the first 30 days: Monday through Friday, then transition to the weekend schedule
• Some in-person work required at clinic, community organizations, and partner locations (mileage and gas reimbursed)

What You’ll Do
• Identify, outreach, engage, and enroll eligible individuals virtually (phone, text, email) and in-person at partner sites
• Build trust with people who may be hesitant or distrustful of healthcare systems due to lived experience
• Explain Enhanced Care Management services and benefits in a clear, patient-centered way
• Educate partners on Pair Team’s services and help link members to the program
• Manage the full enrollment funnel end-to-end, including follow-ups, blockers, and status updates
• Promote timely communication between internal teams and external partners
• Hit enrollment metrics and performance goals tied to mission impact
• Use multiple tools and platforms to track outreach, enrollment stages, and documentation

What You Need
• Must be located in the United States
• Quiet, HIPAA-compliant workspace with reliable internet
• 1+ year experience in a metric-driven or quota-driven role
• Bilingual English and Spanish (reading, writing, speaking, listening)
• Strong communication skills and the ability to connect with people in tough situations
• Comfort using multiple tech platforms and learning new tools quickly
• Valid driver’s license and auto liability insurance
• Community-minded, results-driven, and willing to do repeated outreach without taking it personal
