What You’ll Do
- Manage short- and long-term patient cases over the phone
- Conduct assessments to identify diagnoses, treatment needs, and barriers to care
- Partner with RN Case Managers to develop individualized care plans
- Educate patients about treatment options, benefit utilization, referrals, and care navigation
- Maintain communication with patients, caregivers, providers, and multidisciplinary teams
- Monitor interventions, evaluate progress, and report measurable outcomes
- Build strong relationships to provide support, guidance, and clear information
- Use claims-based tools to assess patient history and determine program eligibility
- Document all case activity accurately and meet quality and turnaround requirements
What You Need
- Graduation from an accredited LVN/LPN program with an active LVN/LPN license
- Experience in case management, care coordination, wellness programs, or related roles
- Background in medical, surgical, or acute care
- Strong understanding of case management principles and health program coordination
- Clear communication skills with the ability to collaborate across teams
- Computer proficiency, including Microsoft Office and Outlook
- Knowledge of medical claims, ICD-10, CPT, and HCPCS coding preferred
- Familiarity with utilization and quality management philosophies preferred
- Ability to assess claims data and develop appropriate nursing treatment plans
- Strong customer focus, time management skills, and organizational habits
