Remote within the United States, excluding AK, ME, DC, NJ, CA, NY, MA, CT, CO, WA, OR, and MN.
Must be able to work late afternoons and evenings; specific days (Mon–Sun) confirmed during hiring.
Expected base pay range: $52,500–$60,000 per year, with total compensation influenced by location and experience.
What You’ll Do
- Serve as a key point of contact for clients, families, and referral sources (hospitals, outpatient practices, schools, agencies).
- Ensure a supportive, positive experience for both clients and external providers throughout treatment and discharge.
- Develop comprehensive, clinically appropriate discharge plans for clients completing care.
- Identify and recommend appropriate aftercare resources using a nationwide provider database.
- Make referrals using each provider’s preferred communication channels and follow up to verify placement.
- Communicate treatment updates to referring providers and answer questions about referred clients.
- Ensure all receiving providers have the clinical information needed to continue care.
- Document all case management activities in progress notes and update internal treatment teams on discharge status.
- Collaborate with clinical, admissions, outreach, and partnerships teams to support coordinated care.
- Meet performance metrics and adhere to Charlie Health policies and procedures.
What You Need
- Bachelor’s degree in health sciences, communications, psychology, social work, or a related field.
- Minimum 2 years of relevant experience in healthcare, ideally in client-facing roles such as case management, discharge planning, referral relations, admissions, or outreach.
- Strong interpersonal skills with the ability to build trust and long-term relationships.
- Metrics-focused mindset with experience working toward clear targets.
- Excellent written and verbal communication skills.
- Extreme organization and attention to detail in a fast-paced environment.
- Authorized to work in the United States with native or bilingual English proficiency.
- Comfort learning quickly and adapting to evolving processes.
- Proficiency with Salesforce and Google Suite/MS Office.
