Join PharMerica’s Continue CareRx team and help patients access the right care at the right time. We’re seeking an Enrollment Coordinator to guide patients through our pharmacy and care coordination program with compassion and professionalism.

Our Continue CareRx Pharmacy program combines pharmacy, virtual nursing, pharmacist support, and care coordination to improve outcomes for patients at home.
Schedule
- Remote role, must reside in Central or Eastern time zones
- Monday – Friday, 9:00 AM – 5:30 PM ET
- Some travel required (10%)
Responsibilities
- Contact eligible patients to explain and enroll them in the Continue CareRx Pharmacy Program
- Educate patients on program benefits and provide support throughout the enrollment process
- Conduct in-home visits as needed to assist patients requiring additional support
- Track and maintain accurate enrollment records and patient data
- Collaborate with clinicians, leadership, and branch teams to coordinate care and ensure program success
- Monitor enrollment and retention metrics, reporting outcomes and suggesting improvements
- Attend branch director meetings and provide insights on program performance
- Troubleshoot patient or clinician challenges related to enrollment and provide guidance
- Deliver excellent customer service to patients, families, and internal teams
Requirements
- Licensed LPN/LPT/LVN, nationally certified Pharmacy Technician, OR licensed Social Worker (with degree from an accredited program)
- Valid driver’s license, acceptable driving record, and proof of insurance
- 1+ years of medical-related experience preferred
- Home health care experience a plus
- Strong communication skills (oral and written) with the ability to engage patients and collaborate with teams
- Detail-oriented, organized, goal-driven, and customer-focused
- Able to manage multiple priorities and navigate challenges with professionalism
