- Sunday–Thursday, 3:00 PM – 11:30 PM ET
- Tuesday–Saturday, 11:00 AM – 7:30 PM ET
- Tuesday–Saturday, 3:00 PM – 11:30 PM ET
Responsibilities
- Engage members through digital coaching, encouraging program opt-ins and goal setting.
- Use coaching frameworks and motivational interviewing to create and refine SMART goals.
- Conduct introductory and follow-up appointments to build long-term member relationships.
- Provide personalized health education and behavior change strategies.
- Collaborate with providers to update care plans and coordinate care across digital and in-person networks.
- Monitor member health data remotely, escalate as needed, and ensure seamless access to services.
- Anticipate social determinants of health needs and connect members to resources.
- Support cross-functional initiatives, contribute to quality improvement, and cover for teammates when needed.
Requirements
- 1–2+ years of experience in health coaching.
- Certification: NBC-HWC or ICF required.
- Must reside permanently in one of the following states: AZ, DC, DE, FL, GA, HI, IL, KS, MA, MD, ME, MI, MN, NC, NJ, NM, NV, NY, OH, OR, PA, SC, TN, VA, VT, or WI.
- Strong communication skills (written and verbal) with empathy and cultural awareness.
- Track record of achieving KPIs for opt-ins, adherence, and satisfaction.
- Growth mindset and adaptability in a remote, fast-paced environment.
- Tech-savvy; comfortable with new systems and tools.
Preferred Qualifications
- Experience supporting high-risk members in pregnancy, fertility, or maternal-child health.
- Clinical background in nursing or behavioral health.
- Startup or digital health experience.
- Familiarity with Google Suite, EHRs, CRM platforms, and Zendesk.
Compensation & Benefits
- Base salary: $62,000 – $70,000 per year
