🧾 About the Role
Pomelo Care is transforming care for moms and babies through technology-driven, evidence-based support that reduces preterm births, NICU admissions, c-sections, and maternal mortality. As a Care Coordinator, you’ll play a critical role in supporting our clinical team by streamlining operations, improving care access, and delivering high-touch support to patients navigating pregnancy and postpartum care.
You’ll report to the Market Operations Lead and directly impact families across the country—especially those facing barriers to care.
✅ Position Highlights
• $45,000–$50,000 base salary
• Remote role based anywhere in the U.S.
• Full-time schedule: Monday–Friday, 1 PM – 9 PM ET

📋 What You’ll Own
• Help patients find in-network providers and connect them to resources like WIC, SNAP, housing, and transportation
• Manage inbound calls and resolve patient questions with empathy and clarity
• Complete insurance eligibility checks and handle patient offboarding
• Schedule and prioritize appointments while optimizing provider calendars
• Submit and track referrals to health plan case managers
• Collaborate with leadership to refine workflows and boost team efficiency
🎯 Must-Have Traits
• 4+ years of experience in healthcare administration
• Experience in virtual care settings strongly preferred
• Familiarity with social determinants of health (SDOH) platforms and referral systems
• Meticulous attention to detail and commitment to follow-through
• Strong communication skills and comfort working cross-functionally with clinicians
• Passion for maternal and child health
• Calm and flexible in fast-moving or ambiguous situations
