Sharecare is hiring a Forms Completion Specialist to handle Disability and FMLA documentation with precision, speed, and empathy. This is a full-time, remote role where you’ll apply medical knowledge and data entry skills to ensure forms are completed accurately and delivered within strict turnaround times. You’ll be supporting patients and clients with one of the most sensitive and time-critical aspects of their care journey.

πŸ“‹ What You’ll Own
β€’ Complete Disability and FMLA forms based on standardized industry responses
β€’ Extract form requests from EMRs and input data into internal systems
β€’ Ensure accurate ICD-10 codes and relevant medical info are included
β€’ Communicate with patients and coordinators via phone and email
β€’ Navigate various EHR platforms to retrieve and apply accurate data
β€’ Deliver completed forms to patients or clients within 5–7 business days
β€’ Track, document, and follow up on form status and related inquiries
β€’ Ensure HIPAA and HITECH compliance at all stages of the process
β€’ Meet quality, productivity, and turnaround benchmarks consistently
β€’ Assist in identifying process improvements related to forms workflow


🎯 Must-Have Traits
β€’ 1+ year in a medical records or similar health documentation setting
β€’ Familiarity with Disability/FMLA forms strongly preferred
β€’ Working knowledge of medical terminology and ICD-10
β€’ Proficient in Microsoft Word and PDF handling
β€’ Strong written and verbal communication skills
β€’ Ability to type at least 50 WPM
β€’ Highly organized, detail-focused, and process-driven
β€’ Adaptable to changing systems, tasks, and compliance needs
β€’ Motivated self-starter with proven customer service skills
