📋 What You’ll Own
• Deliver telephonic and omni-channel support to members with chronic conditions
• Guide members through goal-setting, behavioral change, and care planning
• Educate members on preventive exams and disease-specific standards of care
• Coordinate with care managers, dietitians, and coaches to ensure continuity
• Identify and address social and systemic barriers to care
• Review provider treatment plans with members and encourage engagement
• Document all activity and maintain confidentiality in compliance with HIPAA


🎯 Must-Have Traits
• Current RN license (compact multi-state license required)
• Minimum 3 years of recent clinical experience
• BSN preferred
• Strong background in chronic disease education and management
• Experience with motivational interviewing and behavior change models
• Strong critical thinking and problem-solving ability
• Professional, self-directed, and highly organized
• Comfortable working in a fast-paced virtual care environment
• Effective communication with patients, peers, and providers
• Typing speed of at least 40 WPM
