As a Discharge Planner at Charlie Health, you’ll help close one of the most critical loops in behavioral health care—what comes after treatment. You’ll provide treatment updates to referring providers and ensure smooth transitions for clients exiting our program. This role blends coordination, empathy, and problem-solving to set families up for success beyond our virtual doors.

• Full-Time, Remote (U.S. Only)
• Salary Range: $52,500–$60,000/year

šŸ“‹ What You’ll Own
• Deliver timely, high-quality treatment updates to referring providers
• Coordinate with hospitals, schools, outpatient programs, and other sources of referral
• Create and manage comprehensive discharge plans for clients completing treatment
• Identify and coordinate appropriate aftercare services and resources
• Maintain and update referral options in a national provider database
• Ensure receiving providers have all necessary documentation and case details
• Follow up with clients and providers to verify post-treatment placements
• Partner with internal departments—Clinical, Admissions, Outreach—for seamless collaboration
• Achieve performance metrics related to timeliness, satisfaction, and case throughput


šŸŽÆ Must-Have Traits
• Bachelor’s degree in a related field (health sciences, psychology, social work, etc.)
• 2+ years of healthcare experience in roles such as case management, discharge planning, or admissions
• Excellent relationship-building and listening skills
• Proven ability to meet KPIs in a client-facing setting
• Meticulous with documentation and detail-oriented follow-through
• Strong written and verbal communication skills
• Authorized to work in the U.S. and fluent/native English speaker
• Proficient with Salesforce, Google Suite, and/or Microsoft Office
• Able to adapt quickly and prioritize in a fast-paced virtual setting
