Charlie Health is redefining the continuum of mental health care—and as a Continuing Care Coordinator, you’ll serve as a vital link between clients, families, and referral sources. You’ll ensure treatment updates are timely and transitions out of care are seamless. Whether you’re building trust with hospital partners or helping a family plan next steps after discharge, your impact echoes long after a client leaves our program.

Position Highlights
Full-Time, Remote (U.S. Only)
• Salary Range: $52,500–$60,000/year

📋 What You’ll Own
• Deliver treatment updates to referring providers and respond to inquiries
• Serve as a trusted point of contact for hospitals, outpatient clinics, schools, etc.
• Plan comprehensive, clinically sound discharge paths for clients completing treatment
• Use and maintain a national provider database for referrals
• Make and confirm aftercare appointments through preferred provider channels
• Maintain clear documentation on referral communications and case updates
• Partner with Charlie Health’s internal teams—Clinical, Admissions, Outreach—to support success
• Adhere to process standards and hit performance metrics


🎯 Must-Have Traits
• Bachelor’s in health sciences, communications, psychology, social work, or related field
• 2+ years of healthcare experience (case management, admissions, discharge, outreach)
• Interpersonal and relationship-building skills that inspire confidence
• Detail-oriented and highly organized with strong follow-through
• Excellent communication—written and verbal
• Salesforce, Google Suite, and/or Microsoft Office proficiency
• Results-driven and comfortable working toward concrete KPIs
• Authorized to work in the U.S. and fluent in English


💻 Remote Requirements
• Stable internet connection and suitable work environment
• Must reside in the U.S.
• Ability to manage and prioritize independently in a virtual setting
