Schedule
- Remote, overnight schedule
- Shifts: Thursday, Friday, Saturday, and every other Wednesday
- Hours: 7 PM – 7 AM EST
What You’ll Do
- Monitor patient vitals, symptoms, and labs to identify interventions and escalate appropriately.
- Use standardized telephonic triage protocols (e.g., Schmitt-Thompson) to guide patient care.
- Field inbound patient calls and escalate urgent issues to emergency services if needed.
- Conduct follow-ups on abnormal readings and lapses in patient monitoring.
- Document patient case notes in Cadence’s platform and manage follow-up tasks in Zendesk.
- Support rescheduling, outreach, and administrative tasks as needed.
- Deliver every patient interaction with empathy, precision, and alignment with partner health systems.
What You Need
- Active compact multi-state RN license.
- 4+ years of experience treating patients with CHF, hypertension, and diabetes (inpatient or outpatient).
- Strong clinical acumen in triage, chronic disease management, and patient support.
- Ability to operate independently, with sound judgment in urgent situations.
- Passion for patient experience and improving healthcare delivery through digital innovation.
Pay
- $80,000 – $85,000 annual base salary
