ConcertoCare is seeking a Referral Specialist to manage provider orders and referrals, coordinate care, and ensure timely access to services for patients. This is a full-time, remote role aligned to Pacific Time hours.

Schedule

  • Full-time, remote role (must be available 8:00 AM–5:00 PM PT)
  • Reports to Director of Operations
  • Pay: $23–$26/hour plus annual bonus (commensurate with experience)

What You’ll Do

  • Coordinate and manage patient referrals to specialists and external providers
  • Process provider orders for labs, imaging, durable medical equipment, and diagnostics
  • Verify insurance requirements and secure necessary authorizations
  • Track and follow up on outstanding orders and referrals to ensure timely completion
  • Serve as liaison between providers, vendors, patients, and external partners
  • Document referral and order details accurately in the EHR system
  • Educate patients on the referral process and assist with scheduling
  • Support clinical staff with updates and referral information
  • Identify and address barriers to care such as transportation or scheduling challenges
  • Participate in team huddles, meetings, and organizational initiatives

What You Need

  • High school diploma or GED required
  • 2+ years of experience in a healthcare setting
  • Experience with insurance verification and authorization
  • Proficiency in EHR systems, health insurance portals, and Microsoft Office
  • Familiarity with medical terminology and HIPAA compliance
  • Strong organizational, documentation, and communication skills
  • Ability to work independently and collaboratively in a remote environment

Preferred

  • Certified Medical Assistant or prior MA experience
  • Experience in care coordination, referrals, or value-based healthcare settings
